Outpatient psychotherapy is available to children, adolescents, teens, adults, couples and families. Youth and families graduating from in-home services are encouraged to seek outpatient therapy. Continuation of care is critical to sustainable wellness.
Intensive In-Community (IIC) is a clinical intervention service that addresses emotional and behavioral challenges of youths (ages 4-21) with moderate to high needs. The service is delivered in a youth’s home and in their community, based on therapeutic treatment goals. Services are provided by a licensed therapist using solution-focused interventions. These interventions are intended to stabilize and support the youth and family in an effort to deter more intensive interventions. This process works as a collaboration with the family’s formal and informal supports. The need for this service is determined by the Biopsychosocial Needs Assessment. Caregivers should call Performcare to request a Biopsychosocial Assessment (1-877- 652-7624.) You can request Practical Therapy Network to complete this assessment for your child.
Behavioral Assistance
Behavioral Assistants (BA) deliver specific, outcome-oriented interventions following a treatment plan developed by a Licensed Clinician. BA’s help the youth and family to practice positive coping strategies and techniques. This focus of this service is behavior modification to improve youths overall functioning. Behavioral Assistance is provided in the home and community.
Mentoring
Our mentors serve as a positive role model. Mentors take a creative approach in working with youth in the areas of recreation, social and vocational. Mentors by design help to support, encourage, plan and achieve goals desired by the youth. The mentor can be someone to share ideas, to ask for guidance from and to take part in activities with. Mentoring is provided through our relationship with the Care Management Organization (CMO.) Caregivers should call contact their CMO and can request Practical Therapy Network as their provider from mentoring services (Bergen County only.)
Biopsychosocial Needs Assessment (BPS NA)
The Biopsychosocial Needs Assessment supports the determination of eligibility by matching the need to the appropriate level of care. All children entering the Children’s System of Care will have a Needs Assessment completed. This tool will be used to document and communicate the needs of the child and family, to determine the intensity of services needed and serve as a decision support tool.
